Job summary

The Well NE in Norfolk, NE is seeking an ACT Medication Management Provider who delivers psychiatric assessment and medication management within the Assertive Community Treatment program, collaborating with the multidisciplinary team to support recovery and community-based care.

The role participates in shared caseload and after-hours on-call rotation, emphasizes trauma-informed practice and client-centered decision-making, and ensures continuity of care across clinics, home visits, and crisis

Qualifications

• Graduate medical or nursing education appropriate to the provider’s license and scope.
• Current, unrestricted Nebraska professional license and prescribing authority appropriate to assigned services.
• Psychiatry or psychiatric mental health training and experience preferred.

Responsibilities

• Conduct psychiatric assessments and medication management for ACT participants.
• Develop and update medication plans with participants using shared decision-making.
• Coordinate treatment across psychiatry, primary care, nursing, substance use services, hospitals, pharmacies, and community providers.
• Participate in ACT meetings and after-hours on-call coverage.
