Job summary

A C Adult and Child Health, Inc. seeks an Access Screener to facilitate smooth transitions into System services for clients meeting entrance criteria or refer to other providers. You will gather information, communicate with internal and external resources, and ensure timely documentation and data entry.

The role emphasizes welcoming interactions, adherence to timelines, and collaboration with program leadership to meet performance metrics and deliver high-quality service.

Qualifications

  • Bachelor’s degree required.
  • Background check including criminal, driving, abuse/neglect and fingerprint checks required.
  • New Hire Orientation completion at start of employment.
  • Relias training at start and annually thereafter.
  • Current driver’s license and acceptable driving record required.

Responsibilities

  • Determine if System has the appropriate services for client/patient.
  • Gather service needs, demographic, insurance and payer information.
  • Provide information to external resources based on client needs/location.
  • Represent the System and its values in a positive, welcoming manner.
  • Collaborate with community partners and internal service lines to link clients to services.
  • Complete required documentation and data entry accurately and timely.
  • Enter data in the EHR and tracking tools.
  • Monitor referrals and follow up to meet timelines and metrics.
  • Other duties as assigned by Program Leadership.

Job description

Job Summary

The Access Screener assists in facilitating a smooth transition into System services for those who meet entrance criteria or to help ensure a positive experience for those referred to other providers.

Essential Job Functions
  • Obtain appropriate information to determine if System has the appropriate services for client/patient.
  • Gather information regarding service needs, demographic, insurance and/or payer information.
  • Provide information to external resources based on client need/location.
  • Represent the System and System’s values in a positive manner; being friendly, welcoming, and exhibiting a positive attitude at all times.
  • Collaborate with community partners and internal service lines to link client/patient with appropriate services and support; communicate with internal lines and community partners via phone and email.
  • Complete the System’s required documentation and data entry in a clear, accurate and timely fashion.
  • Complete data entry in a clear, accurate and timely fashion in the Electronic Health Record (EHR) and other tracking tools.
  • Ensure established timelines are met by monitoring/following up with referrals and/or staff.
  • Meet established metrics assigned to position: Return of all calls received by 4:00 p.m. of the business day (goal > 95%).
  • Collaborative documentation (goal > 95%).
  • Documentation and follow up of incoming referral (goal 100%).
  • Other duties as assigned by Program Leadership.

Experience and Education Qualifications
  • Bachelor’s degree in Health and Human Services or related field.
Employment Requirements
  • Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.

  • All training requirements including Relias at the beginning of employment and annually thereafter.
  • Current driver’s license, acceptable driving record and current auto insurance.
Physical Requirements

ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
