Responsibilities

Conduct client intakes, comprehensive assessments, and ongoing case management to identify needs, barriers, risks, and opportunities.
Develop and update individualized case plans with defined goals, timelines, and follow‑ups.
Coordinate housing, employment, education, health, and other essential services for clients.
Monitor client engagement, progress toward goals, and changes in circumstances.
Navigate court-related requirements and assist with program transitions and closures.
Document all client interactions in Salesforce and maintain timely reports for stakeholders.
