Responsibilities

• Evaluates patients and develops care plans including modalities, procedures, exercise programs, therapeutic groups, and education.
• Gathers and interprets data from medical records, physician reports, patient, and family.
• Documents evaluations, tests, progress notes, daily treatment notes, and discharge notes as needed.
• Communicates evaluations and progress to physicians, referral sources, and other clinical staff; coordinates schedule and discharge planning.
• Provides patient and family education on deficits, progress toward goals, and home programs including equipment use.
• Directs and supervises activities of Physical Therapist Assistant, PT technician, students, and volunteers.
