BAS Healthcare, Inc. invites a qualified Advanced Practice Nurse Practitioner to join our Fortuna, CA family practice team. You will care for pediatric and adult patients in a federally qualified health center setting, Monday–Friday, 8:00 AM to 5:00 PM.

Requirements include active CA license, DEA, NP Furnishing License with Schedule II, BLS, and Medi‑Cal enrollment or willingness to credential. Credentialing takes ~45–90 days.

Qualifications

• Active California license required.
• DEA registration active prior to start date.
• PAs must have CA Controlled Substance Education Course Certificate.
• NPs must have a Furnishing License with Schedule II.
• BLS certification required.
• Providers must be ORP enrolled in MediCal or enroll during credentialing.

Responsabilités

• Provide comprehensive family practice care to a mixed pediatric and adult patient population.
• 18 patient encounters per shift, Monday through Friday, 8:00 AM - 5:00 PM.
• Chart closure required within 72 hours.

The area provides a peaceful, close-knit community atmosphere with easy access to redwood forests, rivers, and the Pacific coastline.

Facility: This community health center is a federally qualified health center dedicated to providing accessible, high-quality primary care services to a diverse patient population. The facility operates Monday through Friday and serves a mix of pediatric and adult patients in a collaborative and supportive clinical environment.

Opportunity: Advanced Practice - Nurse Practitioner - Family Practice

• Provide comprehensive family practice care to a mixed pediatric and adult patient population
• 18 patient encounters per shift, Monday through Friday, 8:00 AM - 5:00 PM
• Chart closure required within 72 hours
• Must have a minimum of 3 years of experience in a similar clinical setting
• Active California license required; licensed in CA only
• Active DEA required prior to start date
• PAs must have a CA Controlled Substance Education Course Certificate
• NPs must have a Furnishing License with Schedule II
• BLS certification required
• Providers must be ORP (order, referring, and prescribing) enrolled in MediCal or willing to enroll during credentialing
• Candidate CVs must include month and date on all work experience with all gaps in employment accounted for
• Required immunizations prior to start date: Flu Vaccine, TB Status/X-Ray Results (within 90 days of start), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, and Tdap Immunization (within the last 10 years)
• Credentialing timeframe is approximately 45-90 days
• 30-day notice required for ending all assignments

Compensation: Competitive hourly rates with weekly pay and malpractice included.
