Job summary

Canal Pointe Nursing and Rehabilitation Center in Akron, OH is seeking a compassionate RN to join our team on 12-hour shifts (7am-7pm and 7pm-7am). You will provide high-quality patient care, administer medications, and support residents and families in a warm, patient-centered environment.

Current Ohio RN license required; long-term care experience preferred. You will collaborate with an interdisciplinary team to develop care plans, monitor resident changes, and maintain detailed records.

Qualifications

• Current Ohio RN license.
• Experience in long-term care preferred.

Responsibilities

• Provide direct patient care to residents, including medication administration and wound care.
• Develop and implement individualized care plans with the interdisciplinary team.
• Monitor and report changes in resident condition to the nursing team and physician.
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction.
• Maintain accurate and detailed records of patient care and billing information.
• Collaborate with other healthcare professionals to ensure seamless transitions and quality care.
