Aliya Healthcare in Chicago, IL is seeking a dedicated Registered Nurse or Licensed Practical Nurse (LPN) to join our compassionate care team on the night shift. You will provide total nursing care to residents on the assigned unit and supervise team members.

Qualifications

• Graduation from an accredited nursing education program.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an RN or LPN.
• Participate in developing and implementing individualized care plans with the healthcare team.

• Maintain an environment promoting residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and procedures.
• Monitor staff performance and assist with development and corrective action as needed.

Duties and Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as a Registered Nurse or Licensed Practical Nurse (LPN)
• Participate in developing and implementing individualized care plans alongside the healthcare team
• Administer medications and treatments as prescribed and evaluate effectiveness regularly
• Maintain an environment that promotes residents’ physical, social, and psychological well-being
• Educate residents and families on care plans and medical procedures
• Monitor team member performance and assist with development, evaluation, and corrective action if needed
• Delegate and ensure completion of assigned nursing tasks to team members
• Stay current with facility policies, procedures, and ongoing nursing education
• Recognize and act on significant changes in resident conditions
• Document care, responses, and relevant information per facility guidelines
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions
• Follow all facility safety policies and procedures

Skills and Qualifications

• Graduation from an accredited nursing education program
• Active Registered Nurse or Licensed Practical Nurse License with Illinois Departement of Professional Regulations.
