Qualifications

• RN license and state licensure required.
• Reliable, independent transportation to perform home visits.

Responsibilities

• Provide skilled nursing care including teaching, treatments, IV therapy, wound care.
• Assess patient condition and coordinate plan of care with physician and family.
• Supervise LPNs and home health aides and evaluate care every 14 days.
• Document assessments, visits, and care plans; complete OASIS as assigned.
• Meet weekly with supervisor to review patient status.
• Deliver home health services with dignity and compassion.
