Job summary

You will monitor patient conditions, administer medications and treatments, support individualized care plans, and document care while communicating with the health care team. An active LPN license and BLS are required, with various shift options available.

Qualifications

• Active LPN license valid in the state of California.
• Current BLS certification is required.
• 1+ year of nursing experience preferred.

Responsibilities

• Provide direct patient care in the home.
• Monitor patient condition and report changes to the team.
• Administer medications and treatments as ordered.
• Support individualized care plans and document care.
