Job summary

CommonSpirit Health at Home in Cincinnati, Ohio is seeking an experienced Registered Nurse for Home Health to provide exceptional, patient-centered care in the patient's home. You will build trusting relationships, perform thorough assessments, and deliver personalized nursing services that support recovery and independence.

You’ll lead care plans, collaborate with a multidisciplinary team, educate families, and mentor LPNs/LVNs and aides while leveraging technology to document and improve

Qualifications

• Active RN license in the state served.
• Minimum of 1 year of nursing experience; home health a plus.
• Strong clinical expertise and case management abilities.
• CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver high-quality, personalized nursing care in patients' homes.
• Develop and adjust comprehensive care plans.
• Collaborate with multidisciplinary team.
• Educate patients and families.
