This full-time role involves 1:1 patient care in the home setting, coordinating care with the interdisciplinary team, and delivering high-quality nursing services to patients.

Responsibilities include creating care plans, administering medications, educating families, and maintaining accurate documentation.

Qualifications

• RN degree (Associates or Bachelors) with current, unrestricted license in MD.
• One year nursing experience in patient care; home health, geriatrics preferred.

Responsibilities

• Assess/monitor patients' physical, emotional, and clinical needs.
• Develop home health care plans aligned with physician orders and patient goals.
• Administer medications and treatments; provide pain and symptom management.
• Educate families and caregivers; maintain timely documentation.
• Participate in on-call rotation as required by the local branch.
