Adoration Health is seeking a licensed practical nurse (LPN) for full-time home health in the Memphis area. The role involves providing 1:1 patient care in the home, administering medications and treatments, and coordinating with family, case managers, and physicians to ensure optimal outcomes.

The position emphasizes independence with team support, ongoing patient assessment, and adherence to plans of care.

Formación

• Active, unrestricted nursing license in state of practice.
• CPR certification.
• Valid driver’s license with clean driving record.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsabilidades

• Providing 1:1 care for clients following the plan of care at all times.
• Administering medications, feedings, oxygen, ostomy care as needed per plan of care.
• Documenting nursing actions and communications with family, case manager, and physicians.
• Coordinate or order supplies, medications, and feedings as needed.
• Ensure patient needs are met per physician instructions.
