Job summary

You will administer medications, document care, and communicate with the healthcare team to support each patient’s care plan.

Flexible shifts (day, evening, overnight) with options for full-time, part-time or PRN, and dedicated local support to assist you in delivering high-quality home-based care.

Qualifications

• Active LPN license is required.
• Current BLS certification is required.
• 1+ year of nursing experience is preferred.

Responsibilities

• Provide direct patient care in the home.
• Monitor patient condition and report changes.
• Administer medications and treatments as ordered.
• Support individualized care plans.
• Document care and communicate with the healthcare team.
