Job summary

West Texas Centers MHMR is seeking motivated individuals to perform ongoing Case Management for people served. Qualifications

• Must have and maintain a Valid Driver's License and driving record acceptable for insurability.
• Must pass background check and drug screening.

Responsibilities

• Help clients access medical, social, vocational, and educational services.
• Coordinate services across providers and programs.
• Monitor service delivery and progress toward goals.
• Advocate on behalf of clients to obtain needed resources.
• Provide crisis resolution support when needed.
• Engage clients in activities designed to reduce symptoms of mental illness and improve community functioning.

Job Functions include assisting individuals in gaining needed services:

• access to medical, social, vocational, educational
• coordination of services
• service monitoring
• advocacy
• crisis resolution
• engaging clients in activities designed to reduce the symptoms of mental illness and to increase the ability of the individual to function in the community.

Requires flexible hours, involves extensive paperwork, and will require self-directed performance. Travel and the use of a personal vehicle is required.
