Job summary

Southeast Georgia Health System is seeking a Registered Nurse for virtual care across our network. You will conduct remote assessments, triage, and follow-ups via video, phone, or secure messaging, while collaborating with physicians and bedside teams to ensure continuity of care.

Ideal candidates hold an ASN with Georgia RN licensure and 3+ years in acute care, with preferred cardiac/critical care experience. Remote role with strong patient advocacy and adherence to EHR standards.

Qualifications

• ASN is required.
• Current RN license to practice in Georgia.
• Minimum 3 years of RN experience in an acute care hospital with preference for cardiac/critical care.

Responsibilities

• Conduct virtual assessments, triage, and follow-up visits via video, phone, or secure messaging.
• Monitor patient status using telehealth systems and escalate to bedside staff when needed.
• Perform virtual patient assessments focusing on pain, mobility, and discharge readiness.
• Provide evidence-based nursing interventions virtually/remotely.
• Assist with medication reconciliation, admission & discharge planning, and chronic disease management.
• Document all patient interactions in the EHR per hospital standards.
• Provide real-time clinical consultation to staff.
• Support rapid response teams by reviewing data and escalating concerns.
• Coordinate patient care with physicians, bedside nurses, and allied health professionals.
• Support transitions of care between inpatient, outpatient, and community settings.
• Educate patients and families on treatment plans and adherence.
• Advocate for access to virtual care for patients.

• Supporting rapid response teams virtually by reviewing patient data and escalating concerns.
• Partnering with physicians, bedside nurses, and allied health professionals to coordinate patient care.
• Supports transitions of care between inpatient, outpatient, and community settings.
• Educating patients and families on treatment plans, medication adherence, and lifestyle modifications.
• Advocating for patients need to ensure equitable access to virtual care services.
• Providing Care on the Medical Surgical Units when not scheduled for Virtual Care

What you will need:

• ASN Required, Bachelor of Science in Nursing (preferred)
• Current RN License to Practice in the State of Georgia
• Minimum 3 years of RN experience in an acute care hospital. Critical care cardiac experience (preferred) CAVRN within one year of hire or 1000 hours VC
