Job summary

Rock County seeks a qualified professional to provide mobile, person-centered behavioral health care for clients in crisis across the life span. You will assess risk, coordinate admissions and medical clearances, and develop safety plans to prevent re-crisis, while delivering brief psychotherapy and linkage to services.

Job description

Provides mobile person-centered behavioral health care services to clients across the life span who are experiencing a behavioral health crisis. Completes suicide and violence risk assessments. Collaborates with internal and external providers to identify and address immediate safety needs. Facilitates voluntary and involuntary inpatient psychiatric stays when indicated. Provides risk management safety planning and linkage to ongoing services. Provides ongoing supportive services to client who are at risk of re-entering a behavioral health crisis. Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and “Commitment to My Co-Workers.”

60% Provides person-centered behavioral health care services for clients experiencing a behavioral health crisis
  • Assess clients in crisis to determine least restrictive disposition that will assure client safety
  • Assess for voluntary admission options for individuals without a funding source.
  • Authorize chapter 51.15 emergency detentions when appropriate.
  • Coordinate admissions, when clinically indicated, to the appropriate receiving facility.
  • Coordinate medical clearance if required with police, referring facility, and hospital ER staff.
  • Work with individuals and natural supports to create appropriate safety plans when clinically indicated and in consultation with the crisis supervisor.
  • Create person-centered crisis response plans, crisis plans
  • Provide individual, family, and/or group brief psychotherapy
  • Coordinate client care and linkage to ongoing behavioral health services as assigned
20% Consult and assist with recommendations to law enforcement agencies about those individuals who are experiencing an AODA related crisis.
  • Assess for and facilitate detox admission under 51.45
  • Provide post-discharge risk assessments for clients discharging from a detox facility.
  • Coordinate referrals to Rock County Connections Staff for ongoing linkage to services,
10% Conduct correctional detox, suicidal, and homicidal assessments.
  • Provide behavioral health risk assessment at the Rock County Jail and Youth Services Center
  • Coordinate the admission process with facility staff when psychiatric hospitalization is clinically indicated.
5% Ensure program and medical record compliance
  • Follow all departmental policies and procedures
  • Document client care in accordance with policies, procedures, and regulatory guidelines
3% Participates in all clinical and administrative activities as required
2% Will perform other duties as assigned
Professional expertise in the following areas preferred. Training and ongoing professional development will be provided.
  • Knowledge of the principles and practices of behavioral health care and co-occurring disorders, including assessment of risk and safety planning.
  • Knowledge of laws, regulations, and practices pertaining to federal and state human service programs, specifically chapters 34, 51,75, 92, 94.
  • Knowledge of human service and community resources and how to effectively utilize them.
  • Knowledge of Medicaid, Medicare, HMO's, Managed Care organizations, and public assistance programs.
  • Knowledge of psychotropic medications, their main effects, side effects, and abuse potential.
  • Knowledge of psycho-social rehabilitation services
  • Ability to plan and organize work effectively.
  • Ability to take initiative, plan and organize work independently.
  • Ability to manage one's time and emotions in a fast paced often high pressure environment.
  • Ability to maintain accurate, current records, and prepare clear and concise reports from them.
  • Ability to establish and maintain effective working relationships within the agency and community.
  • Ability to understand and carry out verbal and written instructions.
  • Knowledge and application of best practices in cultural competency, person centered care, and Trauma Informed Care.

Bachelor's degree from an accredited college or university with coursework in areas directly related to providing behavioral health services is required.

Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure 5.02 and 5.27.

Physical Elements
  • Ability to stand, walk and sit. Physical movement occurs a majority of the day.
  • Ability to enter client homes without assistance, including the ability to navigate stairs.
  • Participate in disaster evacuations and fire drills when circumstances warrant. Maintain safety procedures.
  • Lifting and carrying up to 25lbs. on occasion.
Equipment Use
  • Ability to use a computer and keyboard.
  • Must become efficient at using a computerized documentation within 2 months of hire including the use of Microsoft Office products, an electronic medical record system, and other web based systems.
  • Ability to use a copy and fax machine as well as a cellphone.
Working Conditions
  • Able to work in multiple locations including the office, consumer's homes, and a variety of community locations.
  • Inside and outside work with exposure to weather conditions.
  • Potential for contact with unpredictable and aggressive persons.
  • Potential for interactions with animals including but not limited to household pets.
