Job description

We are looking for dynamic nurses to develop and provide independent nurse‑led services in chronic disease management, health promotion and preventive care.

Nursing Care
  • Assess, plan and deliver evidence‑based nursing care for chronic conditions patients
  • Develop person‑centred care plans addressing bio‑psychosocial needs
  • Document care accurately and administer medications safely per protocols
Patient Education & Self‑Management
  • Teach chronic disease self‑management (diet, medication adherence, exercise, monitoring techniques)
  • Conduct depression, anxiety and functional assessments
  • Provide health education and support behaviour change
Care Coordination & Collaboration
  • Participate in multidisciplinary team discussions for complex cases
  • Navigate care systems and coordinate with community partners for continuity
  • Monitor patient progression, track outcomes and recall defaulters
Programme Development
  • Support chronic care initiatives and health education programmes
  • Assist in developing clinical protocols and patient education materials
Requirements
  1. Post‑basic qualification in Diabetes Management and Education / Gerontology / Chronic Disease Management. Other related post‑basic qualifications may be considered.
  2. Bachelors Degree in Nursing will be an added advantage
  3. Experience in nursing and managing patients with chronic conditions is preferred
  4. Registered Nurse with Singapore Nursing Board
