Working Days & Hours: Mon-Fri 830am-530pm

Job Description

• To perform home visits to chronic ill/ palliative patients who are being referred
• To perform nursing assessment on the patient
• To coordinate with doctor for medical assessment and follow-up visits to patient
• To monitor current medication and compliance as prescribed by the doctor
• To perform nursing procedures such as wound dressing, insertion of urine catheter, insertion of naso-gastric tube and care of tubings as required
• To initiate nursing care plan and implement new measures of management
• To educate caregivers/families on basic nursing procedures to ensure continuation of care at home

Job Requirements

• Min Diploma/Degree in Nursing with VALID SNB & BCLS
• 3-5 years of nursing experience in acute ward settings, preferably in palliative settings
