Job summary

SingHealth Community Hospitals (SCH) comprises of Sengkang Community Hospital and Outram Community Hospital, Singapore's only cluster of community hospitals. You will be integral to the nursing care team for rehab and sub-acute patients, ensuring continuity of care during hospitalisation and guiding as patients transition home and into the community.

Qualifications

• Diploma or Degree in Nursing or its equivalent.
• Valid Registered Nurse practicing certificate issued by Singapore Nursing Board.
• Valid BCLS certification.
• Comfortable with shift work (3-rotating shift) and deployment within two work sites under SingHealth Community Hospitals.

Responsibilities

• Provide nursing care to rehab and sub-acute patients as part of the care team.
• Facilitate continuity of care during hospitalisation and transition to home and community.
