Job description

Assist Nurse Manager in providing support and quality care services to clients. Responsible to supervising care staff to ensure physical well-being and safety of clients, including performing of basic nursing care to residents which includes monitoring vital signs, personal hygiene, toileting, feeding and ambulating.

Job Responsibilities

Assist in supervision of care staff

Provide care for clients, attending to medical emergency, medication management and administration etc.

Ensure compliance to nursing, infection prevention and control, safety requirements and regulations.

Maintenance of client medical records and medical follow up schedules.

Provide input to clients’ ICP and ensure ICP is executed

Conduct relevant training to staff and health education to clients.

Responsible for the induction of all grades of new nursing and care staff.

Ensure incident report is put up and submit to Nurse Manager within stipulated timeline.

Performs ADLs for residents (e.g. personal hygiene, toilet training, feeding, ambulating)

Bring residents to the hospital or polyclinics to seek medical treatment or for follow-up reviews; reports to doctor on our assessment of the medical condition/behavioral issues of the residents and feedback to Nurse Manager about the doctor’s assessment, prescription and investigation reports

Taking and reporting of vital signs, weight and conduct monthly monitoring of glucose level

Collaborate with the therapists in the implementation of the planned therapy to achieve set goals

Conduct therapeutic and maintenance programme under the guidance of therapists

Engage with residents

Observing, documenting & reporting of residents’ condition

Requirements

Possess a Diploma or Degree in Nursing

Minimum 5 years of Nursing experience

Those with community nursing experience in Nursing Home or Day Care Centres will be advantageous

Strong clinical experience

Able to work independently and manage a team of HCAs and Nursing Aides

Able to conduct training to new nursing staff

Open to working 2 shifts, weekends and public holidays
