Job description
Reporting to: Head Community Care / Senior Social Worker / Centre Manager
Job Purpose

To support the psycho-social well-being of seniors through casework, care coordination, and social support activities, working closely with the Centre team to promote active ageing, social connectedness, and early intervention for vulnerable seniors in the community.

Key Responsibilities
1. Casework and Care Coordination
  • Conduct intake and needs assessments for seniors referred to the AAC.
  • Develop and implement individual care plans in consultation with the Senior Social Worker and Centre team.
  • Provide emotional support and basic counselling to seniors with adjustment, loss, or family-related issues.
  • Coordinate with AIC, FSCs, healthcare providers, and social service agencies for referrals and resource linkage.
  • Monitor and follow up on seniors’ progress and update case notes in the system (e.g. eCase, CRM).
2. Outreach and Engagement
  • Participate in block-by-block outreach and home visits to identify isolated or vulnerable seniors.
  • Assist in onboarding new seniors, ensuring smooth transition into centre activities and peer networks.
  • Support initiatives that connect seniors to preventive health, exercise, and learning programs.
3. Group Work and Program Support
  • Co-develop and co-facilitate therapeutic, support, or educational group activities, e.g. grief support, life review, or social skills groups.
  • Provide input on programs to enhance social inclusion, mental wellness, and intergenerational engagement.
  • Work with volunteers and partners to deliver meaningful activities that strengthen seniors’ purpose and sense of belonging.
4. Case Documentation and Reporting
  • Maintain accurate, timely case documentation in compliance with agency and AIC standards.
  • Assist in preparing periodic reports, data collation, and evaluation of social work outcomes.
5. Professional Development and Teamwork
  • Participate in case discussions, supervision, and multidisciplinary meetings.
  • Attend trainings and courses to strengthen competencies in gerontology, case management, and mental health.
  • Contribute to a supportive and learning-oriented team culture within the AAC.
