Job description

The Communities of Care focus on developing a place-based model of care and building partnerships in local neighbourhoods for the betterment of the seniors. Scope of work could include coordinating social health services and connecting seniors to relevant service providers to enable them to better manage their health or illnesses. Facilitate care for frail and vulnerable seniors by referring them to home care support and match stay-alone seniors with other seniors and volunteers who are able to support them socio-emotionally.

Key Responsibilities
  • Engage with relevant social and healthcare stakeholders to develop and pilot workflows, protocols and processes to facilitate social-health integration
  • Set up linkages and protocols between different community providers to manage clients across multiple social-health services
  • Review and track the effectiveness of inter-agency workflow in facilitating care transition and provision
  • Develop and implement new initiatives to meet the social and health needs of the community.
Qualifications
  • Bachelor/Graduate Diploma in Social Work or relevant fields (Psychology, Community Work)
Requirements
  • At least 2-3 years of relevant working experience with good knowledge in eldercare or healthcare setting would be advantageous
  • High proficiency in written and spoken English with good facilitation and presentation skills
  • Strong interpersonal and communication abilities with individuals and systems at multi-levels
  • Team player with an ability to function effectively in a fast-paced environment.
