Job Summary

Purpose of job

To provide case management and counselling services to individuals with mental health conditions, dementia as well as support for their caregivers.

Key Responsibilities Areas

• Conduct client and caregiver assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop and implement individualised care plans and interventions.
• Provide case management, counselling and care coordination for clients with mental health conditions.
• Prior experience or knowledge in elderly mental health will be an advantage.
• Plan, organise and implement community outreach, programmes and group work.
• Collaborate with community partners and stakeholders to support clients with complex health and social needs.
• Deliver talks to raise awareness of mental health and dementia.
• Support and manage volunteers who are befrienders to clients with mental health needs.
• To undertake any duties as assigned by the Head, Community Mental Health and the Executive Director.

Requirements

Experience:

• Experience and/or knowledge in working with clients on Mental Health challenges and Person-Centred Care and recovery will be an advantage.
• Experience in case management, group work and community work will be an advantage.
• Strong interpersonal and communication skills.
• Able to work independently and and collaboratively within a team.
• Good administrative and computer skills with proficiency in MS Office

Grad Diploma/ Degree in Social Work

Others:

Willingness to travel and conduct home visits across various locations.
