Key Responsibilities & Duties
Casework Engagement & Assessment
  1. Develop professional and engaged relationship with clients and other key service stakeholders.
  2. Conduct needs and risk assessment during intake by taking into account the integration of a range of information independently.
  3. Participate in case management supervision and consultations.
Casework Case Review & Documentations
  1. Prepare social reports, session notes and ensure documentations are in place.
  2. Submit timely reports to supervisor(s), management and stakeholders where necessary.
  3. Liase with external agencies to obtain client’s social, medical and other relevant documentation.
Casework Goal Setting & Intervention Plans
  1. Develop case management intervention plans for clients.
  2. Provide counselling and support services to clients and their families using a systemic framework and other modalities.
  3. Work closely with multi-disciplinary team and/ or other agencies to deliver care/ case plans.
  4. Co-lead/lead case conferences as part of a multi-disciplinary team.
Group Work, Talks and Workshops
  1. Lead or co-lead therapeutic group work for targeted groups, i.e. support group, therapy group, task group.
  2. Lead or Co-lead psychoeducational and/or support groups.
  3. Be involved in group work planning and facilitation
  4. Develop, plan and conduct relevant talks / workshops for clients and the public.
  5. Prepare group work reports and evaluations.
Community Work
  1. Conduct environmental scanning and assessment of community needs.
  2. Initiate, design, develop and implement new community outreach or development initiatives by mobilising community resources, other social service providers and volunteers.
  3. Organise and evaluate community outreach and development programmes, taking into account the broader perspectives.
Professional Development & Education
  1. Seek out continuous learning opportunities such as participating in case review meetings, practice research and development to further develop skills and capabilities.
  2. Abide by professional Social Work Code of Ethics; identify and respond to ethical concerns in practice.
Programme Development & Implementation
  1. Advocate for and co-lead the development of specific programmes targeted at service gaps and service delivery issues.
  2. Design programme and conduct outcome-based evaluation to ensure that programmes are able to support the needs of gaps identified/ key clients.
  3. Assist in the management of the end-to-end development and delivery of programmes.
Research
  1. Participate and support research efforts with guidance.
  2. Engage in multi-disciplinary team research projects.
Others
  1. Prepare company projects or report submissions, as assigned by Supervisor.
  2. Contribute towards fundraising efforts of agency.
  3. Carry out any other duties as assigned by HOD / Supervisor.
