Key Responsibilities & Duties
Casework Engagement & Assessment
- Develop professional and engaged relationship with clients and other key service stakeholders.
- Conduct needs and risk assessment during intake by taking into account the integration of a range of information independently.
- Participate in case management supervision and consultations.
Casework Case Review & Documentations
- Prepare social reports, session notes and ensure documentations are in place.
- Submit timely reports to supervisor(s), management and stakeholders where necessary.
- Liase with external agencies to obtain client’s social, medical and other relevant documentation.
Casework Goal Setting & Intervention Plans
- Develop case management intervention plans for clients.
- Provide counselling and support services to clients and their families using a systemic framework and other modalities.
- Work closely with multi-disciplinary team and/ or other agencies to deliver care/ case plans.
- Co-lead/lead case conferences as part of a multi-disciplinary team.
Group Work, Talks and Workshops
- Lead or co-lead therapeutic group work for targeted groups, i.e. support group, therapy group, task group.
- Lead or Co-lead psychoeducational and/or support groups.
- Be involved in group work planning and facilitation
- Develop, plan and conduct relevant talks / workshops for clients and the public.
- Prepare group work reports and evaluations.
Community Work
- Conduct environmental scanning and assessment of community needs.
- Initiate, design, develop and implement new community outreach or development initiatives by mobilising community resources, other social service providers and volunteers.
- Organise and evaluate community outreach and development programmes, taking into account the broader perspectives.
Professional Development & Education
- Seek out continuous learning opportunities such as participating in case review meetings, practice research and development to further develop skills and capabilities.
- Abide by professional Social Work Code of Ethics; identify and respond to ethical concerns in practice.
Programme Development & Implementation
- Advocate for and co-lead the development of specific programmes targeted at service gaps and service delivery issues.
- Design programme and conduct outcome-based evaluation to ensure that programmes are able to support the needs of gaps identified/ key clients.
- Assist in the management of the end-to-end development and delivery of programmes.
Research
- Participate and support research efforts with guidance.
- Engage in multi-disciplinary team research projects.
Others
- Prepare company projects or report submissions, as assigned by Supervisor.
- Contribute towards fundraising efforts of agency.
- Carry out any other duties as assigned by HOD / Supervisor.
Requirements
- Bachelor/Postgraduate Diploma in Social Work
- Good interpersonal and teamwork skills.
- Demonstrate sound and updated professional knowledge and skills in social work.
- Good analytical skills.
- Meticulous and efficient
- Effective leadership skills.
- Ability to work in a team.
- Knowledge of Microsoft Office
- Excellent communication skills
