Job summary

NHG Health is seeking a Social Work Associate to support the Acute Inpatient, Community Hospital, Outpatient and Community settings. You will provide psychosocial care to patients and families, including financial assistance, working with a multi-disciplinary team to create holistic care plans.

We welcome Diploma holders who are team players and flexible, with the ability to rotate across different settings and support the MSW in casework management and department administration.

Qualifications

• Diploma holder, preferably with relevant working experience
• Team player who enjoys working in a fast-paced environment
• Open to job rotation across Acute Inpatient, Community Hospital, Outpatient and Community settings

Join our dynamic and vibrant department as Social Work Associate in KTPH & YCH and make an impact on the lives of patients and their families. We are seeking compassionate and trained individuals to support our acute, community hospital and outpatient clinics.

As a SWA, you will be responsible for providing psychosocial care (including financial assistance) to our patients. Working closely with multi-disciplinary team to establish a holistic care plan and carry out appropriate interventions are essential aspects of the work as a SWA. You are also required to provide support to the MSW in the casework management and department admin work such as managing front counter duties.

Qualifications:
