Job summary

Celecti Pte Ltd is seeking a dedicated Social Worker to join our team in Singapore. You will manage case referrals, collaborate with the centre lead for assessments and interventions, and monitor the delivery of coordinated services to clients.

Responsibilities

• Manage case referrals.

• Adopt a suitable case management approach when working with clients.
• Refer and coordinate care services to ensure clients are adequately supported.
• Keep abreast of, and provide information on, the various assistance schemes and care options available.
• Assess family financial status.
• Manage the administration of grants, subsidies and support services where relevant.
• Ensure all MOH service requirements on financial and quality aspects are adhered to.
