Overview

Job Summary

• execution of casework support under the guidance of a social worker, including home visits to engage clients and caregivers living with mental health conditions.
• Support basic intervention tasks identified by social workers.
• Engagement of clients with dementia with meaningful activities through befriending and cognitive stimulation programmes.
• Monitoring of client’s conditions and progress.
• Provide service linkage for client and caregivers.
• Support the team by providing administrative, operational and logistical assistance for client-related services, programmes, outreach efforts and group work activities.

Key Responsibility Areas

• Passion to work with individuals and caregivers with mental health conditions.
• Basic befriending, elder sitting duties and emotional support for elderly.
• Support seniors and caregivers during service duration.
• Perform basic mental health screening tools for clients and caregivers.
• Support community outreach, group work and referrals.
• Ensure proper documentation of case notes, assessment, records.

Requirements

• Experience:
  • Prior experience in working with elderly clients and caregivers in the community setting.
  • Relevant working experience in healthcare or social service sector.
  • Able to co-facilitate group work.
