Job Description:

The Social Work Assistant (SWA) gathers corroborative information to support accurate assessments and facilitate appropriate service referrals. The SWA is to connect patients with suitable community resources and contribute to a team providing comprehensive care that addresses both medical and psychosocial needs. SWAs may accompany MSWs on home visits to gain deeper insights into patients' circumstances. Beyond direct patient interaction, the SWA is required to support administrative tasks like organising meetings, tracking outcomes, and data collation. The SWA’s work is essential in ensuring patients receive necessary financial aid and community support, while also contributing to the efficient operation and development of the Medical Social Services in SHP.

Requirements:

  1. GCE O or A levels with 6 years of experience in the social service sectors or customer service experience; or
  2. Diploma in Social Sciences (Social Work)/ Psychology Studies/ Counselling/ Community Care or Services/ Social Service/ Social Enterprise or its equivalent
  3. Able to speak local languages
  4. Proficient in Microsoft Office applications
