Location: East region of Singapore

We are looking for a compassionate and dedicated Palliative Care Nurse to join our multidisciplinary team. In this role, you will provide holistic, patient-centered care to individuals with life-limiting illnesses, focusing on symptom management, comfort, and emotional support for both patients and their families.

Key Responsibilities
  • Deliver comprehensive nursing care to palliative patients in accordance with established clinical guidelines and care plans.
  • Assess, plan, implement, and evaluate individualized care, ensuring patients’ physical, emotional, and psychosocial needs are met.
  • Manage symptoms such as pain, breathlessness, nausea, and anxiety effectively.
  • Provide emotional and psychosocial support to patients and families, fostering comfort and dignity throughout the care journey.
  • Work collaboratively with doctors, allied health professionals, and social workers as part of a multidisciplinary team.
  • Educate and counsel patients and caregivers on disease progression, medication management, and end-of-life care options.
  • Participate in case discussions, care conferences, and quality improvement initiatives.
  • Ensure accurate documentation and maintenance of patient records.
  • Adhere to infection control, safety, and ethical standards in all aspects of care delivery.
Requirements
  • Diploma/Degree in Nursing or equivalent.
