Location: West (5 days, Clinic Hours)

Role Overview:

Provide nursing support in a specialized outpatient service focused on diabetes management. Responsibilities include:

• Assess patients’ lifestyle, diet, and social factors to provide personalized coaching to better manage diabetes.
• Collaborate closely with physicians to develop and implement individualized care plans.
• Educate patients on insulin use, blood sugar monitoring (including technology-assisted methods), and interpreting glucose data, both in outpatient and inpatient settings.
• Support quality improvement projects within the clinic to enhance patient outcomes.
• Perform other tasks as assigned.

Qualifications:

• Registered Nurse with recognized diploma or equivalent.
• Specialist certification in diabetes education preferred but not required.
• Minimum 1 year experience in chronic disease or diabetes management.
