Job description

Reporting to the Nurse Manager / Nurse Clinician, you will be responsible for delivering patient‑centric and compassionate care according to patients’ needs and conditions.

Responsibilities

• Monitor, reassess, and evaluate the patient's condition, implementing a care plan for the patient and supporting family members to cope with the patient's advanced illness.
• Communicate and build rapport with patients and their families.
• Maintain proper documentation of the patient's condition and accurate collation of data.
• Perform nursing care, such as medication administration and wound dressing, as required.
• Follow up to ensure continuity of patient care and treatment plans.
• Maintain a high level of confidentiality and discretion in matters related to patient care.

Requirements

• At least a Diploma in Nursing from the local Polytechnic and registered with the Singapore Nursing Board.
• Able to perform rotating shift duties.
• Good interpersonal and communication skills.
