Qualifications

Responsibilities

• Assess patients’ condition and implement nursing care plans.
• Perform nursing procedures such as wound dressing.
• Ensure continuity of treatment plans for patients.
• Maintain accurate documentation and report to primary care provider.
• Administer medications under physician's orders.
• Monitor patients' pain levels at end-of-life.

As a Senior Staff Nurse (Home Care Setting), you will be responsible for the following duties:

• Provide basic care such as checking and recording vital signs, maintaining patients' hygiene and assisting with daily activities like dressing or eating.
• Assessing patients’ condition and implement nursing care plan.
• Performing nursing procedures such as wound dressing and management, removal of stitches, venepuncture, changing of urinary catheter and etc.
• Ensuring continuity of treatment plans for patients.
• Maintain accurate documentation of procedures carried out and medications dispensed.
• Reporting to patients' primary care provider.
• Offer a professional source of comfort to the patients and their families.
• Explain the patients' condition or symptoms, listen to their concerns and relay information between the patient and their family.
• Administer certain medications to patients under the orders of a physician.
• Monitor patients' pain levels at end of life, and continue to offer solutions to physical discomfort, including pharmaceuticals and /or therapies.
• Guide their patients through end-of-life emotions and are there to support patients when family is not available.
