• Performing nursing procedures in patient's home such as wound dressing and management, removal of stitches, venipuncture, changing of urinary catheter and etc.
• Ensuring continuity of treatment plans for home care patients.
• Maintain accurate documentation of procedures carried out and medications dispensed.
• Any other ad-hoc duties within the team when necessary
