Job description
Clinic Management – Hemodialysis (HD)
  • Support Nurse Manager and deputize in the absence of Nurse Manager on the following:
  • Plan, lead, organize and control the business and operation of clinic. This include prudent financial management, performance for smooth operations and effective use of the resources at the clinic including operating and capital equipment budget process.
  • Responsible for the performance in the Clinical Balance Score Care and KPIs (BSC).
  • Collaborate in the development of long term business plan for the clinic.
  • Organize and hold regular meetings for the clinic to facilitate open communication and teamwork.
  • Responsible for Incidents Reports (relating to clinical, operational and technical issues, patients’ feedback and grievances) by performing investigation and escalation to Nurse Managers, Clinical Manager, ADON, Operation Managers and Medical Director in a timely manner.
  • Plan and prepare staff rostering ensuring full optimization of staff efficiency versus patients’ numbers and compliance with agreed nurse patient ratio.
  • Ensure that dialysis stations are managed and utilized to the maximum.
  • Mange the expectations and relationship between doctors, clinics and patients.
  • Collaborate with Nurse Manager in developing, implementing and auditing nursing standards.
  • Allocate staff and equipment as necessary to ensure adequacy of resources to meet care requirements.
  • Maintain cooperative working relationships with other departments and member of the health care team.
  • Ensure all equipment and consumables meet standard requirement, at optimal working condition and safe for patient’s use.
  • Assume the duties of other staff in the clinic in the event of staffing shortages.
  • Assume additional duties and responsibilities assigned by Nurse Manager.
Provision of Hemodialysis (HD) Care
  • Provide doctor-in-charged with up-to-date information of patients’ clinical data and findings for the effective and appropriate intervention. This includes follow doctor consultation round and carry out doctor’s order.
  • Participate and assist actively during doctor’s consultation round. Follow-up on doctor’s order.
  • Provide regular structured and ad-hoc health education and counselling to patients, their family and care givers as a means to improve the quality and safety of care.
  • Practice primary nursing and plan treatment plan for assigned patients. Develop short and long term goals specifying expected outcome. Communicate treatment plan of patient to other patient care providers. Implement treatment plan, monitor progress and review outcome regularly.
  • Demonstrate competency in dialysis-related procedures according to protocols including the care and procedures of dialysis access.
  • Preparation, commencement and completion of dialysis treatment, management of dialysis complications etc.,
  • Responsible in assessing and documenting patient’s vital signs and psycho-physio-social needs before, during and after treatment. Apply knowledge of altered health status in assessment process. Report any abnormalities to Nurse Manager timely, inform doctor and initiate referrals to hospitals and community resources when necessary.
  • Adhere to proper aseptic techniques and infection prevention and control protocol.
  • Administer medications, blood and blood products safely according to protocol. Demonstrate knowledge of action, dosage and side effects of medications.
  • Demonstrate competency in collection of patient blood sample, analyze blood results, update doctor and counsel patient. Timely reporting to Nurse Manager for any abnormalities and inform doctor is necessary.
  • Demonstrate competency in collection of water sample and water quality test. Monitor water quality and report abnormality to Technical Service and Nurse Manger timely.
  • Responsible to clear dialyzers and bloodlines after dialysis and perform dialyzer reprocessing procedures according to protocol (if applicable).
  • Demonstrate competency in dialysis equipment related procedures according to protocol.
  • Including troubleshooting, machine disinfection, care and maintenance of machine tec.,
  • Demonstrate competency in general nursing procedures eg. Dressing, resuscitation procedure, blood glucose monitoring, checking and replenishing of emergency trolley and equipment.
  • Document care provided accurately and timely and ensures confidentiality of patient’s information.
  • Ensure safety of patient at all times. Report and record any incidents to Nurse Manger timely.
  • Assume additional duties and responsibilities assigned by Nurse Manager.
Professional Development of Staff
  • Collaborate with Nurse Manager to conduct Orientation Program to train new staff (with/ without renal experience) to assist new staff to progress from probationary to expert in the care of dialysis patient.
  • Provide guidance to existing staff for clinical and personal development leading to staff satisfaction and retention.
Professional Development of Self
  • Assume responsibility for own professional competence by demonstrating willingness to evaluate own nursing practice.
  • Participate in continuing education programs to gain current knowledge of development.
  • Maintain relevant membership in professional colleagues and public.
  • Comply with required dress code.
