Job summary

RESPONSIBILITIES

Direct Patient Care
-
1. Provide independent clinical assessment of patients and formulate individualized plans of care within the scope of practice for a Registered Nurse and in consultation with the multi-disciplinary team, referring appropriately for internal or external services as necessary.

2. Perform clinical procedures as required for patient care.

3. Perform on-call duties on a rostered basis to provide out of office hours phone call support to patients and escalating emergent cases to on-call doctor as necessary.

Other Indirect Clinical responsibilities
-
1. Assist Nurse Manager/Assistant Nurse Manager in duties including but not limited to nursing consumables inventory, controlled drug storage/tracking and guiding junior staff nurses in performing their duties.

Competency Development and training
-
1. Completion of required competency training within the stipulated time frame.

2. Track learning hours and proactively look out for learning opportunities to meet development requirements.

3. Assist Nurse Clinician/Assistant Nurse Clinician in duties of clinical audits and clinical supervisions and guiding of junior staff nurses in performing their duties periodically.

Quality Improvement
-
1. Proactively participate in QI and audit projects undertaken by the HC/HH team, taking up the role of assistant project manager if necessary

Any other tasks that may arise as part of MWS HC/HH service requirements.
JOB REQUIREMENTS
- Registered with Singapore Nursing Board
- Minimum 5 years of Nursing work exposure and minimum 2 years of community health care work exposure
- Specialist/Advanced Diploma in Gerontology, Palliative Care or equivalent preferred
- Must be able to carry 10kg luggage bag
- Good verbal and written communication skills
- Good team player
