Qualifications

• 3 to 5 years of relevant experience in mental health and dementia care.
• Demonstrated competencies in case management, group work, and programme development.
• Able to travel and conduct home visits within designated service boundary.

Responsibilities

• Assess care needs of clients and develop individualised care plans.
• Manage COMIT post-discharge programmes in partnership with healthcare institutions.
