1. Senior Social Worker

Working hours: Mon – Fri | 9am – 6pm

Location: Bedok , Chai Cheedrive

Purpose of job

Assess the care needs of clients and caregivers living with mental illness and develop individualised care plans and interventions.

Work collaboratively with healthcare professionals, community partners and families to provide holistic, person-centered care.

Provide supervision and guidance to junior staff in managing casework and support services.

Job Description
  • Manage and coordinate COMIT post-discharge programmes in partnership with tertiary healthcare institutions, including monitoring clients’ progress and programme outcomes.
  • Conduct client and caregiver assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop and implement individualized care plans and interventions.
  • Deliver psychotherapy and conduct mental health talks for clients and caregivers.
  • Build and maintain strong networks with stakeholders and community partners to address service gaps and improve care accessibility.
  • Supervise the COMIT team through individual and group supervision sessions.
  • Promote ongoing training and professional development of team members, aligned with AIC Mental Health Competency Framework.
  • Plan, facilitate and evaluate group work sessions for clients and caregivers.
  • To undertake any duties as assigned by the Head, Community Mental Health and the Executive Director.
Requirements
  • 3 to 5 years of relevant experience in mental health and dementia care, with demonstrated competencies in case management, group work, programme development, and community engagement.
  • Graduate Diploma, Bachelor's, or Master's degree in Social Work, Nursing or Relevant Allied Health Qualifications.
  • Able to travel and conduct home visits within the designated service boundary.

2. Social Worker – Community Interventions

Working location: Tampines

Working hours: 5 days (Mon – Fri), 9 – 6pm

Salary: $3200 - $4400

Responsibilities
  • Initiate and/or participate in outreach efforts related to the Division, which include developing outreach plans to publicise the Division Programmes and establishing good relationships and contacts with stakeholders or community partners.
  • Management of referrals, and engaging referral agency / person for intake and assessment.
  • Case assessment and conceptualisation by conducting a thorough assessment of the case using information gathered during the intake and subsequent engagement with client, family and stakeholder.
  • Collaborate with clients to develop intervention plans, and provide counselling and support to address their risks and needs issues; advocate for clients' rights and access to essential services within the community and relevant systems.
  • Documentation of all case related records.
  • Research and evaluation through keeping abreast of current research and trends, and participate in and support research projects.
  • Professional development and education by preparing for, attending and presenting cases at Internal Case Reviews / Case Conceptualisation meetings / Clinical supervision on a regular basis. Involve in supervising interns and/or students on placement.
  • Develop and implement new projects/initiatives that are aligned with the objectives of the Division.
Requirements
  • Degree in Social Work or equivalent with 2 years of experience in social service sector or similar setting.
  • Well versed with MS Office programmes and experiences with social work interventions; interest in growing clinical skills.
