A healthcare advisory company in Singapore is seeking an experienced professional for mental health and dementia care. Responsibilities include assessing care needs, managing post-discharge programmes, and supervising a team. Candidates must have 3-5 years of relevant experience and a degree in Social Work, Nursing, or Allied Health. Working hours: Mon – Fri | 9am – 6pm Require OT and occasional Saturdays

Purpose of job:
Assess the care needs of clients and caregivers living with mental illness and develop individualised care plans and interventions.
Work collaboratively with healthcare professionals, community partners and families to provide holistic, person-centered care.
Provide supervision and guidance to junior staff in managing casework and support services.
Job Description
Manage and coordinate COMIT post-discharge programmes in partnership with tertiary healthcare institutions, including monitoring clients’ progress and programme outcomes.
Conduct client and caregiver assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop and implement individualized care plans and interventions.
Deliver psychotherapy and conduct mental health talks for clients and caregivers.
Build and maintain strong networks with stakeholders and community partners to address service gaps and improve care accessibility.
Supervise the COMIT team through individual and group supervision sessions.
Promote ongoing training and professional development of team members, aligned with AIC Mental Health Competency Framework.
Plan, facilitate and evaluate group work sessions for clients and caregivers.
To undertake any duties as assigned by the Head, Community Mental Health and the Executive Director.
Requirements
3 to 5 years of relevant experience in mental health and dementia care, with demonstrated competencies in case management, group work, programme development, and community engagement.
Graduate Diploma, Bachelor's, or Master's degree in Social Work, or
Nursing or Relevant Allied Health Qualifications
Able to travel and conduct home visits within the designated service boundary.
Experience with case management practices for individuals with mental illnesses and dementia.
