Qualifications

Duties & Responsibilities
  • Ensure that work done by Social/Case Workers are in accordance to code of ethics under (SASW) and guidelines and standards under (MSF).

  • Supervises the Social/Case Workers work done on individual and family counselling and support for residents.

  • To enhance and ensure that Social/Case Workers discuss long-term care issues with the resident and family.

  • To plan and manage case conferencing for residents.

  • To check on Social/Case Workers conducting needs and risk assessment of each resident as well as reviews periodically using CANS assessment.

  • To ensure that Social/Case Workers provide supervised counselling services including helping residents to identify problems and possible solutions.

  • Collate the needs of Social/Case Workers in providing resolution and referral services for residents who require specialized psychological follow-up.

  • To lead the Social/Case Workers in completing Review Committee 3 and 6 reports, progress reports and ECRM reports.

  • To guide the Social/Case Workers in partnering with appropriate community resources to develop ways to meet identified needs.

  • To lead Social/Case Workers in conducting needs assessments (i.e. residential placement, finance, safety).

  • Ensuring that Social/Case Workers assist residents with attaining resources that fit their needs including the provision of financial assistance when required.

  • To check on Social/Case Workers planning and implementing appropriate interventions.

  • To lead Social/Case workers in conducting financial assessments and maintain all relevant records pertaining to casework intervention with residents and their families.

  • Any other work assigned by Head of Home.

Requirements
  • Graduate Diploma / Degree in the field of social work from a recognized university.

  • 3 - 5 years of related experience.

  • Willingness to learn new skills and effective communication skills with the elderly would be an asset.

  • A team player and able to organize and execute programs.

  • Possesses good attitude with good interpersonal skills.

  • Good work ethics, organized and punctual with strict adherence to Home and Jamiyah policies.
