Job Purpose

Singapore Red Cross operates the Red Cross Home for the Disabled (RCHD), the nation’s only adult residential home for those with moderate/severe physical and intellectual disabilities. We care for those who cannot care for themselves 24/7 around the clock. With therapeutic and recreational activities that stimulate the residents’ body and mind, we take care of their physical, mental, social and emotional well-being.

Job Responsibilities

Social Work/Case Management

  1. Responsible for conducting/renewing means tests and financial assessment.
  2. Establish rapport and build professional relationships with clients and caregivers.
  3. Conduct initial screening and intake assessment to ascertain the level of complexity, key risks and protective factors of the case and prepare for necessary intervention that includes information and referral to help clients and caregivers.
  4. Prepare case history information, biopsychosocial assessment and social work intervention plans. Implement and follow up on care / case plans and interventions to determine the suitability of care / case plans in addressing client’s needs.
  5. Plan, organize and lead admission/discharge planning and actively follow up with parents, RCHD’s multidisciplinary team and external stakeholders with regards to the care plan.
  6. Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the residents, the goals to be accomplished and the appropriate social worker intervention.
  7. Assist residents and their families/significant others with attaining resources that fit their needs including the provision of financial assistance when required.
  8. Communicate and work closely with family members or caregivers for the residents’ basic needs, care plan, etc.
  9. Prepare Multi-Disciplinary Team (MDT) Meeting and recommend/highlight crucial cases if needed to the team. Develop individual care plans and carry out regular review of the cases.
  10. Conduct Client Assessment Form-Revised (CAF-R) evaluations for residents/clients for both ADH and DAC.
  11. Prompt updates and documentation in the SW Communication Log Sheet for purposeful contacts.
  12. To keep appropriate case recordings and produce timely reports, as required, in accordance with departmental policies and procedures.
  13. Liaise with hospitals, government agencies and other community partners relating to the residents’ care plans.
  14. Handle assigned administrative duties and reports.
  15. Carry out any other duties as assigned by the supervisor.

Professional Development & Supervision

  1. Participate in developmental programs and supervision for personal and professional growth & development in accordance with the National Social Work Competency Framework (NSWCF).
  2. Covers Senior Social Worker (SSW) during long leaves.

Stakeholder Management

  1. Build and sustain collaborative working relationships with key ministries and social service partners such as MSF, SG Enable, AIC, hospitals, special needs schools, service providers in the disability sector, and community partners.
  2. Handles any walk-in audits from HMC, BOV, etc.
  3. Understand and meet all government requirements for social service documentation.

Administration

  1. Attend HMC meetings and assist/cover SSW to prepare the agenda, the minutes of meetings, and the executive summary report.
  2. Assist SSW in the preparation and submission of various periodic reviews pertaining to statutory submission and audits.
  3. Handles the various social work reports: SW Monthly Report, SG Enable monthly data updates, etc.
Job Specifications

Qualification and Experience

  1. Recognised Degree/Postgraduate Diploma in Social Work.
  2. At least 5 years working experience in the social service sector and healthcare sector.
  3. Good knowledge in government means testing framework.
  4. Good case management skills.
  5. Good knowledge and applications of various assessment frameworks and approaches.

Additional Skills and Knowledge

  1. Experience in administering and managing Audit Musters would be an advantage.
  2. Ability to speak a few languages or dialects would be an added advantage.
  3. Familiar in using Google Suite.
