Job summary

WECRUIT PTE. LTD. is seeking a care support role to assist in developing and implementing individualized care plans for residents, ensuring progress is monitored and changes are reported to the appropriate personnel.

• Assist in developing individualised care plans for residents according to their assessed needs and the Home’s established procedures.
• Support the implementation of care plans, monitor residents’ progress and report changes or concerns to the appropriate personnel.
• Organise and implement programmes and activities to promote residents’ well‑being, engagement and quality of life.
• Conduct monthly resident interviews and maintain records, including six-monthly review reports, for approximately 50 residents.
