Job summary

Job description
  • Manage clinical caseload and support palliative rehabilitation practice (e.g. breathlessness management, prescription of equipment, collaborative goal setting) across the various care settings.
  • Deliver evidence-based individualised care through holistic comprehensive assessment, treatment and evaluation of care.
  • To work collaboratively with multi‑disciplinary colleagues to meet patient needs.
  • Participate in service review to drive service improvement.
  • Contribute to the implementation and monitoring of adherence to relevant clinical guidelines.
  • To be a resource and support to other team members and community professionals.
  • Take initiative in pursuing life‑long learning and personal development through reflective practice and engage actively with learning and development opportunities.
  • Supervise and assign appropriate treatment plans to therapy assistants.
  • Maintain high quality patient documentation and records.
  • Any other duties as and when assigned.
Requirements
  • Degree in Occupational Therapy from an accredited institution
  • Must be fully registered with Allied Health Professions Council
  • At least 4 years of experience in an acute hospital setting or community setting
  • Passion to serve

  • Team player with good interpersonal and effective communication skills
  • Able to multi‑task and work in a fast‑paced environment
