- Manage clinical caseload and support palliative rehabilitation practice (e.g. breathlessness management, prescription of equipment, collaborative goal setting) across the various care settings.
- Deliver evidence-based individualised care through holistic comprehensive assessment, treatment and evaluation of care.
- To work collaboratively with multi‑disciplinary colleagues to meet patient needs.
- Participate in service review to drive service improvement.
- Contribute to the implementation and monitoring of adherence to relevant clinical guidelines.
- To be a resource and support to other team members and community professionals.
- Take initiative in pursuing life‑long learning and personal development through reflective practice and engage actively with learning and development opportunities.
- Supervise and assign appropriate treatment plans to therapy assistants.
- Maintain high quality patient documentation and records.
- Any other duties as and when assigned.
Requirements
- Degree in Occupational Therapy from an accredited institution
- Must be fully registered with Allied Health Professions Council
- At least 4 years of experience in an acute hospital setting or community setting
- Passion to serve
- Meticulous and good time management
- Team player with good interpersonal and effective communication skills
- Able to multi‑task and work in a fast‑paced environment
