Job summary

A healthcare organization in Singapore seeks compassionate Senior/Staff Nurses for its Population Health Community Health Team. Qualifications

  • At least 2–4 years' experience as a Staff Nurse in a clinical setting.
  • Experience or strong interest in palliative care is highly desirable.
  • Willingness to work beyond regular hours including evenings and weekends.

Responsibilities

  • Perform comprehensive assessments of nursing home residents.
  • Develop and implement individualized care and treatment plans.
  • Provide clinical guidance and training to nursing home nurses.
  • Facilitate Advance Care Planning discussions with residents.
  • Support staff in delivering high-quality end-of-life care.
  • Participate in quality improvement projects.
