A leading healthcare institution in Singapore is seeking a Social Worker to support patients and families with psycho-social issues. Responsibilities include counseling, coordination with healthcare staff, and maintaining case records. The ideal candidate should have a degree in Social Work, preferably with honors or a master's, and possess strong counseling and communication skills. Experience in social work is an advantage.
Interviews and counsels patients and their families to help them with their psycho-social problems.
Communicates with medical staff, nurses, other health-care professionals to discuss plans for total patient management.
Liaises with other government departments, social services organisations or private agencies to meet patients' needs.
Maintains case records on patients.
Participates in the orientation of new staff.
Represents department on committees of statutory, voluntary and community organisation providing social services.
Attends supervision session, department meetings, case conference and training workshops and seminars.
Carries out psycho-social research and relevant special projects.
Visits patients\' homes to assess social conditions and family dynamics, counsels and gives support
Degree in Social Work or equivalent with preferably with Honours degree or Masters.
Preferably as Registered Social Worker (RSW) with Social Work Accreditation Board.
Prior experience in social work or the social agency setting is preferred.
Good counselling and communication skills.
