Roles & Responsibilities

You are entrusted with the responsibility to establish partnerships with internal and external stakeholders to deliver holistic care experience for seniors. This role involves taking the initiative in collaborating and coordinating with all stakeholders to journey with seniors through their golden years, providing a seamless care experience.

Responsibilities
  • Lead the setup of Integrated Care Community Partners (ICCP) by spearheading the planning and operational capabilities to integrate the services and workflows across all service providers
  • Lead partnership engagement and nurturing relationships to maintain a strong network to support a collaborative and fully integrated care model.
  • Oversee projects with multiple external stakeholders to deliver a seamless integrated care model for seniors
  • Establishing standardization and optimization of processes across various organizations.
  • Assist in compliance monitoring of care plans and subsequent steps of care journey of seniors.
Requirements
  • Bachelor's Degree with 3-5 years’ experience in healthcare, elder care and / or social service.
  • Certification in project management or healthcare administration will be an advantage
  • Possess excellent organisational, problem solving and conflict resolution skills.
  • Excellent communication and interpersonal skills for effective stakeholder management.
