Qualifications

Responsibilities

Lead clinical caseload management for inpatient and outpatient cases, including assessment, intervention, and discharge planning.
Provide expert consultation and clinical leadership for complex, high-risk cases.
Drive quality improvement initiatives to enhance patient care outcomes and service standards.
Collaborate with multidisciplinary teams to deliver safe, person-centred, evidence-based care.
Lead, supervise, and support a team of Medical Social Workers and Social Work Assistants/Associates.
