Job description

Support and assist the nurses (Registered Nurses (RN) and Enrolled Nurses (EN) in the delivery of nursing care in the wards and helps to maintain a clean, safe and comfortable environment for them.

Responsibilities

• Execute ADL tasks eg feeding, showering, transferring, pottering of residents in the ward
• Work with NMS and SNs in the supervision of HCA teams in the wards.
• Training and mentoring junior and Foreign HCAs/SCAs in infection control procedures
• Take on responsibility of ward safety and cleanliness in wards
• Co-Lead in therapy and Social activities with Therapy and Volunteer teams
• Support Volunteers coming to do activities with residents
• Perform some simple assessments of resident’s well being and clinical status (vital signs measurements) under supervision of Nurses and MSW/ Care coordinators
• Communicate with NOKs on resident’s progress if assigned by Care coordinators
• Help Care coordinators track progress on resident’s social Care Plans, booking of NOK appointments for TCUs
• Lead in Work Improvement/Productivity Projects in the wards if any
• Handle transport coordination with Operations team
• Help Care coordinators to manage the residents’ activity calendar
• Attend to needs of the residents. Ensure the safety, comfort and well-being of the residents and reports residents’ need to the nurse
• Ensure the comfort and good personal hygiene of the residents under her/his care, e.g. bed bath/shower bath, changing of clothes and bed sheets, keeping them clean and dry
• Accompany residents for their outing trips
• Alert nurses to emergency situations

ADDITIONAL DUTIES

• Conduct some AIC wellness programs with residents in the wards
• Carry out any other duties as assigned by nurses
