A health services company in Singapore is seeking an experienced professional to support the planning and operationalization of Health Precincts. The role involves developing community health initiatives and engaging residents to promote health ownership. A Bachelor's degree and at least 5 years of experience are required, along with strong planning and stakeholder engagement skills. This position offers an opportunity to make a significant impact in community health management.
Support the planning and operationalization of Health Precincts to drive health–social integration with both internal and community stakeholders.
Develop and execute initiatives that empower the community to take ownership of their health, with a focus on preventive care.
Design and implement Community Health Plans within assigned precincts, ensuring alignment with overall population health goals.
Monitor, track and evaluate the implementation progress of community health initiatives across key focus areas.
Build and maintain strong partnerships with grassroots organisations, social service agencies, community partners and healthcare providers to support integrated care across settings.
Engage residents and patients through community outreach to encourage healthy living and enable better care navigation.
Provide secretariat support for meetings, including agenda preparation, minutes recording and follow-ups.
Undertake any other duties, projects or assignments as delegated by the Reporting Officer and/or Head of Department.
Bachelor degree in any discipline
Minimum 5 years of working experience
Able to think critically
Strong planning, project management and organisational skills
Adept in stakeholder engagement to foster buy-in and collaboration
