Job description
Responsibilities
  • Support the planning and operationalization of Health Precincts to drive health–social integration with both internal and community stakeholders.

  • Develop and execute initiatives that empower the community to take ownership of their health, with a focus on preventive care.

  • Design and implement Community Health Plans within assigned precincts, ensuring alignment with overall population health goals.

  • Monitor, track and evaluate the implementation progress of community health initiatives across key focus areas.

  • Build and maintain strong partnerships with grassroots organisations, social service agencies, community partners and healthcare providers to support integrated care across settings.

  • Engage residents and patients through community outreach to encourage healthy living and enable better care navigation.

  • Provide secretariat support for meetings, including agenda preparation, minutes recording and follow-ups.

  • Undertake any other duties, projects or assignments as delegated by the Reporting Officer and/or Head of Department.

Requirements
  • Bachelor degree in any discipline

  • Minimum 5 years of working experience

  • Able to think critically

  • Strong planning, project management and organisational skills

  • Adept in stakeholder engagement to foster buy-in and collaboration
