Overview

As a member of Planning and Development Department , you will play a key role in shaping this ecosystem—bringing internal and community stakeholders together, coordinating initiatives, and integrating health and social support systems to enhance the delivery of community care and improve health of the population. Job Responsibilities

• Support in planning and operationalizing of Health Precincts concept to achieve health-social integration with internal and community stakeholders
• Support in developing and operationalizing plan to empower community to take care of their health.
• Develop and implement community health plan for the precincts under your care, with emphasis on preventive health and social-health integration
• Track and review progress on implementation of community health plan for different focus areas
• Engage & build partnership with internal and community stakeholders e.g. grassroots organizations, social service agencies and healthcare providers to keep patients and residents healthy and integrated care across care settings
• Any other duties, projects or assignments by Reporting Officer and/or HOD

Requirements

• Bachelor degree in any discipline.
• Minimum 5 years of working experience.
• Strong planning, project management and organizational skills.
• Adept in stakeholder engagement to foster buy-in and collaboration.
• Exceptional written and verbal communication skills
• Resourcefulness in problem solving and resilient in overcoming challenges.
