Job summary

Ang Mo Kio-Thye Hua Kwan Hospital Ltd is seeking a Resident Physician for its Home Healthcare Team to manage clients' acute and chronic medical conditions through regular home visits and telehealth services.

You will work closely with a multidisciplinary team to develop holistic care plans, engage clients and families in care decisions, and perform bedside procedures as needed, including titration of medications and referrals to other services.

Qualifications

• Possess a basic medical degree or postgraduate qualification that is registrable with the Singapore Medical Council (SMC).
• Full registration with the Singapore Medical Council.
• In active clinical practice.

Responsibilities

• Provide medical consultation through regular home visits or telehealth service.
• Work closely with the multidisciplinary team to develop holistic care plans for clients and optimize their medical conditions and functional status.
• Engage clients and/or next-of-kin to develop a care plan that aligns with the clients’ preferences and goals of care.
• Perform bedside procedures and basic management of tubes and drains as required.
• Titrate and prescribe medication for the clients.
• Assist with referrals to Home Nursing, Home Therapy or external social services as needed.
• Ensure documentation of medical records and provide medical reports or referrals when required.
• Attend CME teaching sessions and workshops to keep abreast of medical knowledge.
• Provide teaching to Family Medicine residents if required.
• Other duties as assigned by supervisor.

Responsibilities

• Provide medical consultation through regular home visits or telehealth service
• Work closely with the multidisciplinary team to develop holistic care plans for clients and optimize their medical conditions and functional status
• Engage clients and/ or next-of-kin to develop a care plan that is aligned with the clients’ preferences and goals of care
• Perform bedside procedures such as venipuncture, urinary catheterization, and management of simple tubes and drains
• Titrate and prescribe medication for the clients
• Assist with referral to other internal services eg Home Nursing or Home therapy or external social services if required
• Ensure accurate documentation of medical records
• Provide medical reports, memos, referrals if required
• Attend continuing medical education (CME) teaching sessions and workshops/conferences to keep abreast of medical knowledge
• Provide teaching to Family Medicine residents if required
• Other job-related duties as assigned by supervisor

Requirements

• Possess a basic medical degree or postgraduate qualification that is registrable with the Singapore Medical Council
• Full registration with the Singapore Medical Council
• In active clinical practice
• Possess relevant experience in home care services would be an advantage
• Able to work independently
• A team player with a passion to care for the elderly
• Possess good interpersonal and communications skills
• Willing to travel to clients’ homes to provide services
